REQUIRED MEDICAL

Duchesne Academy requires that ALL students have an annual physical exam and up to date immunization records on file in order to attend school (this includes returning students).  These forms must be signed by the physician and parent or guardian and are due in the Registrar's office by May 31, 2008.

These forms require Adobe Acrobat Reader be installed on your computer in order to open and print.

Students who are not covered by health insurance must have on file with the school a completed and signed Release Form For Students Without Health Insurance. If your child is not covered by a health insurance plan, please print out the .pdf file below and return it to the Registrar by May 31, 2008.

ADMINISTRATION OF PRESCRIPTION MEDICATIONS DURING SCHOOL HOURS
